New to all of this, but have found that when wearing my cage, the head sometimes pokes out the end a bit and then rubs on my underwear. It's like a pin prick and really sucks. If it's not that it feels as if my underwear is pushing the ring back into my pelvis and that sucks as well. Anyone else have/had these issues? How do you correct them? What is the "best" underwear?
Yes, I sometimes experience the first scenario you describe. I have found the best way to avoid this is by not wearing stuff that is too tight. If things are pulled up high and tight, then sometimes you feel that little pinch at the tip of your penis which is interacting with the cage and your underwear.
What type of cage do you have? If it's one of the ones with two vertical bars in the front, you can squeeze them together with a pair of pliers to make it smaller.
I’ve found that the smoother the fabric is let’s things kinda slide around. I have a pa piercing as well which tends to catch on the zipper of my pants
A micro zip-tie (residue clipped off) placed on the front bars to stop anything poking through helps.
This is only a short term solution and prevents clean peeing, but you can use tape. Rip three pieces (or two pices where one is signficiantly smaller in width) at least, with a small one just big enough to cover the opening with the sticky face facing outwards (so it doesn't stick to your skin) and the other(s) enough laid ontop wrapping around the penis opening to keep the smaller tape sealing the internals. The plastic clear type tends to do well with many showers (although tends to discolour over time, especially when dust gets in the air pockets), which you can probably purchase for 20-50 cents a roll. I don't recommend keeping your device longer than 1 day with this technique.

I've found that using a small ankle sock as a pouch isolates him and obviates need for underwear. Agree need to avoid tight pants.
Wacoal b-fitting bikinis are nice and stretchy pima cotton spandex and keep things snug without "scrunching".
