I hope somebody can give me some advice. I am wearing intermittently a Mature Metal Jail Bird cage, 2.5 inches long. It seems my correct size, perhaps I can shorten it a little more. However, while it’s not a problem under very loose pants, like sweatpants, it is under regular pants like jeans or trousers: the tighter fit causes the pants to push the cage sideways or down to the point that the base ring pulls my testicles too much, causing discomfort and pain on the long term. Anybody has some advice?

I had to stop wearing men's undies and my wife switched me to panties. They fit snug and hold my cage (a JB) up. She's bought me some straps as well. She likes me "pointing" straight now. 
The weight of the JB will naturally pull it down, coupled with one ball smaller we have going, the JB will always want to go to the smaller ball side.

My straps came from Amazon. My wife likes the way I'm pointing now. They work for holding us up and tight to our body
i think its a shame for men in trousers cos if i wear jeans it is not as comfy as wearing dresses. in dresses and skirts it dont hurt at all.
I had the same exact MM cage when I started. I am also familiar with your problem after 15 years of wearing chastity cages of all styles and sizes. What you may like to try are tight underwear. I wear a tight boxer underwear from Duluth Trading that is also cooling. It holds everything against my body so pants are not a problem. Surprisingly some types of woman's panties did a good job of preventing your problem. I have many types of panties and men's underwear and when I am home I do not wear underwear, I go commando. When we go out I wear my colling tight fitting boxer underwear.
If course you can just buy looser fitting jeans. I no longer wear tight pants. I have altered my life to accommodate our chastity sex life for the last 15 years. I use lots of silicon lube, choose my underwear and pants accordingly for comfort. You can try what I suggested or chose fashion over chastity.
BTW, I stopped using my MM cage for a 3D printed one that is so comfy as to allow me to wear anything now. It is lightweight and more compact. It also holds my penis as a good angle so that I rarely have an issue with any pants. Good luck.
