I'm taking some tentative first steps into chastity. I got a cobra knockoff from Amazon and, after some getting used to things, found it fairly comfy - aside from some issues with having a foreskin.
I didn't try to do any over-nights but did a few full days in a row. At that point I was getting a very dull pain in one testicle. It took a couple of days after leaving the device off for it to fully subside but it did go away.

For context, I had this once a couple of years ago, was prescribed some antibiotics and checked for anything sinister before it eventually went away. Also context, since I've seen others mention this in relation to similar pain, I have had a vasectomy.

So this experience has left me feeling shy about jumping back in. My question is, can I solve this through better sizing?
Did I just experience epididymal hypertension (descriptions of that suggest that it would go away after ejaculation and this didn't immediately) and it's not a sizing issue at all?

Regarding sizing, I don't really understand what to do with ring size. The cage I got comes with 4 ring sizes which is quite nice for trying things out. The largest is the most comfortable but I had a testicle escape with that one once. The size down is what I was using when I experienced this pain. But I could get all 4 on, with increasing difficulty.
I'm also curious about whether it's a gap issue, and whether this cage puts pressure on the epididymis over time or something. I'm generally "high and tight" if that's a factor.

I really enjoyed the times playing with this and would like to do more. I'm a bit anxious and paranoid about medical things anyway, and would be so mortified by having a self-inflicted medical issue from something kink-related. I'll welcome any advice to help me get back in.
 
I have the same issue. I've found with my MM Jailbird that twisting the base ring post results in the base ring being slightly twisted, providing a bit more room on the impacted side. This has helped quite a bit, but the issue is not totally resolved. Too tight a ring makes it worse. Too lose and it slips off, or a nut pops through.
 
I have the same issue regarding vasectomy and epididymus issues. I also have a minor vericocele on one side. Nope, no solutions. If you try to wear any device long term (more than a couple days), you'll have issues, regardless of fit. I got to the point where any sort of arousal caused pain to the point of conditioning me against arousal, leading to complete loss of libido and thus loss of all interest in chastity or anything like it. So it's essentially self-defeating for me. I can use it as a short term toy, basically, but nothing more.
